It shocked me a bit that Mr. Gaziano thinks that (sic) "bespoke shoes appeal only to a certain kind of gentlemen like those with problem feet or who are shoe obsessive". He is in a better position to know, but I don't share that point of view. Maybe his phrasing was unfortunate.

His comments are consistent with how he treats his loyal repeat MTO clients. As long as your MTO design changes do not require a pattern change, he'll accommodate you...especially if he likes your design.
He lets the client decide if bespoke is appropriate bespoke based on their budget and needs (fit or special design...the shoe obsessive)

Hectorm, you are right, "obsessive" may not be the word here... He is not dealing with the obsessive - those of us just phantasize on a website, in front of a shop window or all in our minds: designs, lasts, leathers... Apparently harmless.
Perhaps "compulsive" is it: "oh, I HAVE TO have a pair of shoes like that!" - that's when Mr. Gaziano intervenes.
Ideally, they combine.
But I'm not saying it is a bad thing...
