Shirt fit: front panel ripples

On some of my MTM shirts, there is a ripple from the first button below the collarbutton to the shoulder/s. Why does this happen?

I have the same problem w/ some custom shirts. My first guess was that the shirt front had a bit too much fabric at the top, but now I think the shirt may be too narrow in the chest.

My guess is that either the shoulders were cut too high and too sloped or that your shoulders have pitched forward than your shirt takes account of

Every time I see that on a customer of our MTM shirts, the collar is not attached correctly or doesn't match the neck hole, or both!
The patterns for our shirt are not made from scratch, they use existing patterns and change them. If they cut out the neck hole incorrectly(too deep in the back and and the shoulders) the collar doesn't fit. The shirt will be pulled to the neck by force, creating some ripples.
Check this by laying the shirt flat on a table. The neck hole should ly almost flat! Something close to this(hopefully not unpressed like my shirt):
