I am thinking of ordering a 4x2 DB blazer. Should it button with a long lapel like this:

That's a nice look if you have the waist for it.

Should it always button at the lowest button? Any examples of a 4 button blazer with a lapel roll to the top button?

Both styles of buttoning have their history. The classic style of the original navy Blazers had the buttoning at the top button, so it could be said that this was the original style. But once the buttoning at the bottom was developed in suits and later in semi-formal evening wear (black tie) in the 20s, it passed on to blazers in the 30s. There are many arguments for both, the choice should be decided by the wearer's will and nature.

What about pockets at this blazer. Did it have patch pockets?

I've found this picture in Cunnington's book on English Costumes. Not a DB, but still an argument for patch pockets to be the classic pockets on a classic blazer?
